To benefit from Simply Cash Plan cover, we require a Direct Debit payment from you every month. The reference that will appear on your bank statement is Simplyhealth. Simply complete your details in the form below. Simplyhealth will not hold any bank details unless you complete your application and join Simplyhealth.

If you're not the account holder, or more than one person is required to authorise debits from the account, we can't complete your application online. Please call us on 0800 197 6999 and we'll send you a Direct Debit form to complete, alternatively you can print off the blank Direct Debit Instruction.

 

Instruction to your bank or building society

Please pay Simplyhealth Direct Debits from the account detailed in this instruction subject to the safeguards assured by the Direct Debit Guarantee. I understand that this instruction may remain with Simplyhealth and if so, details will be passed electronically to my bank or building society.

Banks and building societies may not accept Direct Debit Instructions for some types of account.

Direct Debits can now be set up on-line or over the telephone in addition to the original postal method. All the normal Direct Debit safeguards and guarantees apply. No changes in the amount, date or frequency to be debited can be made without notifying you at least 10 working days in advance of your account being debited. In the event of any error, you are entitled to an immediate refund from your bank or building society. You have the right to cancel a Direct Debit Instruction at any time simply by writing to your bank or building society, with a copy to us.

To set up a Direct Debit online you must be able to confirm both of the following statements:

The Direct Debit Guarantee

  • This Guarantee is offered by all Banks and Building Societies that accept instructions to pay Direct Debits.
  • If there are any changes to the amount, date or frequency of your Direct Debit, Simplyhealth will notify you 10 working days in advance of your account being debited or as otherwise agreed. If you request Simplyhealth to collect a payment, confirmation of the amount and date will be given to you at the time of the request.
  • If an error is made in the payment of your Direct Debit, by Simplyhealth or your Bank or Building Society, you are entitled to a full and immediate refund of the amount paid from your bank of building society.
    - If you receive a refund you are not entitled to, you must pay it back when Simplyhealth asks you to.
  • You can cancel a Direct Debit at any time by simply contacting your Bank or Building Society. Written confirmation may be required. Please also notify us.
